PAIN MANAGEMENT-Minor Pain

Conditions such as headache, myalgia, chest pain, pharyngitis, otitis media, arthralgia, sunburn, strains, and sprains often produce minor pain in children. For treatment of this mild pain, aspirin, acetaminophen, ibuprofen (nonnarcotics), and codeine (narcotic) are excellent oral analgesic medications. Aspirin is one of the oldest analgesic medications, but its use has recently declined. It has the advantage of being inexpensive, and it has anti-inflammatory effects. Sustained high dosages are needed, however, for the anti-inflammatory effect. Thus, in most instances, aspirin is not much better than non–anti-inflammatory drugs.

Aspirin may be given every 4 hours at a dosage of 10 to 15 mg/kg per dose. Higher dosages increase only the anti-inflammatory effect—not the analgesic effect. Buffered aspirin may be tolerated better and absorbed faster, but there is no evidence to show that it acts more rapidly or lasts longer than the nonbuffered variety.

Do You Know About Blood Vessels?

The blood vessels include the arteries, arterioles, veins, venules, and capillaries. Arteries are the vessels that carry blood from the heart; veins are vessels that carry blood to or toward the heart. Both types of vessels are composed of three coats in differing proportions : tunica intima, tunica media, and tunica externa.

The tunica intima, the smooth inner lining, runs uniterrupted throughout the vascular system and thus forms the lining of all the vessels; variable amounts of elastic tissue and smooth muscle occur in the other two coats. The larger blood vessels posses tiny vessels (called vasa vasorum) within their walls for nourishment.

Dermis Vascular System

The dermis contains a complex vascular system arranged in three plexuses. Many structures are supplied retrogradely or have their own vascular and nervous plexuses, e.g., the hair follicles. The varying morphology of pathologic changes is due to certain pathologic processes being principally associated with a particular stage of the vascular system and consequently taking place largely in a particular layer of the skin.

Skin blood vessels are difficult to distinguish in histological sections. The major veins have special wall structures in various skin regions to meet the particular physiological requirements of say the arm or leg. Contraction of the venous wall may cause the tunica muscularis to appear thick so that the vessel looks like an artery to the inexperinced eye. In addition, the venous tunica muscularis in an elderly subject may be substantially thickened, especially in the lower leg. The distribution and structure of the vessels differ according to body region.
