The pancreas contains isolated groups of endocrine cells known as the islands of Langerhans. The beta cells of these islands produce insulin, a protein hormone that regulates carbohydrate metabolism, specifically the utilization of glucose. Extensive or complete failure of the islands of Langerhans to secrete insulin cause the disease diabetes mellitus, which is characterized by thirst, enhanced appetite, urinary frequency, high blood glucose, weakness, emaciation, and the appearance of sugar in the urine. Obesity and heredity are predisposing factors. In at least a fourth of the case there is a family history of diabetes.
The discovery that insulin can be extracted from the pancreas of animals and that it ensures the proper use of glucose made possible the successful treatment of diabetes. In persons in whom the pancreas produces nearly enough insulin, the condition may be controlled by a modification of the diet.

The unbound drug is that fraction of drug in plasma or tissues that is no bound to plasma or tissue proteins. A reversible equilibrium normally exists between the unbound drug and the protein bound drug. Only the unbound drug is considered to be pharmacologically active and in equilibrium with the receptor site. This fraction appears to be responsible for the dose-related therapeutic effects and adverse reaction. The unbound fraction is also available for excretion and elimination.
Certain drugs have a marked hepatic uptake that is capable of rapidly shifting the equilibrium from bound to unbound drug. This creates the effect of so called protein stripping by the liver. For these drugs increased protein binding results in enhanced hepatic clearance. Changes in their rate of metabolism depend mainly on hepatic blood flow. For other drugs an increase in unbound drug generally leads to increased hepatic metabolism. The rate of metabolism of such drugs is normally independend of liver blood flow. Protein binding limits glomerular filtration but generally does not influence renal tubular secretion, which also appears to strip bound drug.

There are, understandably, a number of possible complications of pregnancy, especially in teenage pregnancies. Among others, these include certain infections, anemia, trimester bleeding, hyperemesis gravidarum (pernicious nausea and vomiting), eclampsia (coma and/or convulsive seizure), erythroblastosis fetalis (Rh factor incompatibility), ectopic pregnancy (implantation outside the uterine cavity), and spontaneous abortion (miscarriage).
By convention, abortion is defined as delivery or loss of the product of conception before the twentienth week of pregnancy. About 35 % of women bleed or have cramping sometime during the first 20 weeks of pregnancy; about 20% actually abort. Since in 90 % of spontaneous abortions the fetus is either absent or grossly malformed, and in an additional 5% it has chromosomal abnormalities, spontaneous abortion may be a natural, purposeful rejection. If a woman has three or more successive pregnancies that end in spontaneous abortion, she is said to have habitual abortion.
